Outdoor sculpture survey questionnaires, 1991-1993.

Questionnaires compiled by field surveyors as part of a "Save Outdoor Sculpture" program cover the state outside the city of Chicago. The survey was coordinated by the National Institute for the Conservation of Cultural Property and the Smithsonian Institution's National Museum of American Art. For each surveyed sculpture, the questionnaire provides work's title or alternate title; primary artist; foundry or fabricator; execution date; media; dimensions; markings or inscriptions; owner or administrator; environmental setting; physical condition; overall description; background information (e.g., newspaper clippings; photographs); surveyor name; and survey date.
